The health facility authorises Saybert to disclose relevant information to potential and current staff members and agency members for purposes useful to Saybert regarding employment placements;

The applicant has received a copy of Saybert's current rate schedule and agrees to terms and conditions agreed upon registration and acceptance from Saybert.
